Read this first: This form is to be used by students who anticipate meeting all criteria necessary for admittance into the Funeral Service program at Milwaukee Area Technical College (MATC) for the Fall 2025 semester. This includes all coursework indicated by the Funeral Service Program Program Courses Document.

Upon completion of this petition form, the students will be evaluated by the Funeral Service program advisor and responded to regarding their status. All students will be notified by March 30th of their status.

The petition window is as follows: February 15 – March 10, 2025 (Completed required general coursework as of August 2025)
