Step 1: Gather Disability Documentation
- Documentation must:
- Be concluded by a licensed professional, qualified to diagnosis and treat specific disability.
- Include a clear diagnosis and description of functional limitations.
- Clearly identify each individual diagnosis and related functional limitations when multiple disabilities or barriers are determined.
- Be current and documented on letterhead or official organization reporting materials, as well as signed and dated by diagnostician (documentation should be less than three-years old).
Step 2: Documentation Forms and Guidelines:

- Attention Deficit Disorder and Attention Deficit Hyperactivity Disorder Documentation Release of Information Form
- Psychological Disability Documentation Release of Information Form
- Visual, Hearing, Medical and/or Mobility Impairment Documentation Release of Information Form
- Reasonable Accommodation Request Form/Release of Information Form
