If you are pursuing your GED or HSED and are interested in receiving accommodations, please follow the below steps to request accommodations.
Step 1: Gather Disability Documentation
Documentation Forms and Guidelines:

- Request for Testing Accommodations Learning and Other Cognitive Disorder
- Request for Testing Accommodations Physical/Chronic Health Condition
- Request for Testing Accommodations Psychological and Psychiatric Disorder
Documentation must:
- Be concluded by a licensed professional, qualified to diagnosis and treat specific disability.
- Include a clear diagnosis and description of functional limitations.
- Clearly identify each individual diagnosis and related functional limitations when multiple disabilities or barriers are determined.
- Be current! Suggested:
- Learning Disabilities: <5 years old
- ADHD: <3 years old
- Psychiatric/psychological: <1 year old
- Physical/chronic health conditions: Generally < 1 year old, depending on the condition and its expected duration
- Report must include:
- Relevant information about the history of your condition, its impact on your functioning, what treatments you are using, and your prognosis (how long your condition is expected to continue)
- All scores, subtest scores, and Index scores for any tests that were administered to you
- A specific diagnosis
- Include specific recommendations for testing accommodations, with a rationale for each recommended accommodation (see NOTE # 2 below)
- The signature of the evaluator
Step 2: Request Reasonable Accommodations

Step 3: Schedule Meeting with Accommodation Specialist

Step 4: Develop Accommodation Plan
This consists of meeting with an Accommodation Specialist to review documentation and determine accommodations.
